I do prefer you to schedule your appointment here on my website, it keeps me organized. I do check all emails and appointment bookings on a regular basis throughout the day so last minute request are ok. After I have screened you I will then call you or give you my cell number for further correspondence.

Please fill out the information below and after that I will contact you in order to confirm the appointment and give much needed info, for example, my location which is usually in the North Dallas Galleria area.

I accept Date-Check, and Preferred411 (see below), if you are a member of one of these screening services please include ID # into reference provider section. If you are not a member then please list two current provider references with contact information that can be verified. This information can be entered into the messages window below the "contact
information".

Please understand that if your references are from providers themselves (which you have included email address or phone number) that this usually takes time for a response back and last minute appointments are not possible for you.

I also require a safe phone number to contact you by. Please indicate the date and amount of time that you would like to spend with me.

I have been getting TOO many cancels and NO SHOWS, PLEASE PLEASE pick a time when you know you will have no conflicts in timing.
